HOW DO I....
OBTAIN A REPEAT PRESCRIPTION?

PLEASE GIVE 48 HOURS’ NOTICE OF YOUR REQUEST, ALLOWING FOR WEEKENDS AND BANK HOLIDAYS. HAND IN OR POST YOUR COMPUTER SLIP ENCLOSING AN SAE IF REQUIRED.

IT IS OUR POLICY THAT WE WILL NOT ACCEPT REPEAT PRESCRIPTIONS OVER THE TELEPHONE.


As we are a rural practice, the local pharmacies are very happy to help those patients who find the above methods difficult. By letting the pharmacy have your repeat prescription request direct, they will contact the surgery and will let you know when you can collect your medicines from the pharmacy.

HOW DO I....
OBTAIN SICKNESS CERTIFICATES?

RULES FOR THE CERTIFICATION OF ILLNESS

If you read this it may prevent unnecessary visits to the surgery! If you are sick for less than six days, you are not required to do anything apart from inform your employer (if you have one). If you are sick for six days or longer you may be able to claim either Sickness Benefit (if self-employed or unemployed), or Statutory Sick Pay (if you have worked for an employer for long enough). Sickness Benefit can be claimed using form SC1 available from DSS offices and Statutory Sick Pay using the form SC2 available from your employer. Both these forms cover you until the seventh day of illness. If you are sick for seven days or more, you will have to be seen by the doctor to obtain a sickness certificate. Each subsequent certificate will need to be obtained by seeing a doctor until you return to work (although your GP may provide a certificate without seeing you if a hospital doctor has been treating you).

NB: Some employers insist on a sickness certificate for illnesses lasting less than seven days. As this is not a statutory requirement, it is considered to be a private arrangement and as such there is a charge (sometimes the employer will reimburse this). If you are ill for 28 weeks or more continuously you may be eligible for benefits. Please contact your local Department for Work and Pensions.

REGISTER?

We welcome new patients to our surgery: the area we cover can be seen on the map page. When registering, you will be asked to complete a brief questionnaire and will be given an appointment with our healthcare assistant for a 'new patient' medical.

HOW DO I....
MAKE AN APPOINTMENT?

Patients may see the doctor of their choice by appointment. Urgent cases will be seen without appointment, although this may involve some waiting and the doctor of their choice may not be available. Feverish children brought to the surgery will be seen as soon as possible. PLEASE REMEMBER, EACH APPOINTMENT IS FOR ONE PERSON AND ONE PROBLEM ONLY. If your problem is complex or you have more than one problem, please request a longer appointment when booking.

If you are too unwell to come to surgery, please telephone BEFORE 10.00 AM with full details of your illness. Please also leave a telephone number at which you can be contacted. THE DOCTOR WILL ASSESS URGENT CALLS IMMEDIATELY.

Doctors will visit patients at home at the discretion of the doctor. However, we prefer patients to be seen in surgery. Four patients can be seen in surgery in the time it takes to do one home visit.

CHANGE MY PERSONAL DETAILS?


Please advise the surgery of any change of name, address or telephone number as soo as possible to ensure your medical records are kept up to date.
