How to Request an Appointment
For urgent same-day and routine appointments please complete the online Patient Triage.
You can access the Patient Triage here.
The Patient Triage form is usually available Monday to Friday:
- 08:30am to 12:00pm
- 14:30pm to 16:00pm
Please note: For patient safety reasons. We may close the online Patient Triage form earlier if we reach our daily capacity.
We use a secure online service called Accurx.
We aim to contact you back about appointment requests in 24 hours.
We will contact you back about admin requests within 7 working days.
We will offer you either a same-day appointment or a routine appointment. Routine appointments are in 4 to 6 weeks time.
With the information you provide, we may refer you to other local services. Such a service may be a local pharmacy.

Completing the Form on someone’s behalf
You can complete the Patient Triage form for a family or friend on their behalf. Select the option “Someone’s behalf” before completing the form. This will ask your details and the patient details.
